[Required] Name, contact (WhatsApp / LINE / phone), date of birth, nationality, areas of interest, preferred consultation date and time, language.

[When entering via LINE] LINE User ID (optional).
[Additional, only when actual treatment proceeds] Address, marital status, referral path, foreigner registration number (foreigners only), medical and family history, and any other information deemed necessary by medical staff for medical service.
[Method] Automatic collection through this booking application page.
2. Purpose of Collection and Use
• Provision of services for diagnosis and treatment
• Verification for medical service / examination / appointment lookup
• Billing, payment, refund and other administrative services
• Sending invoices, receipts, certificates, medications and results
• Securing communication channels for handling complaints and grievances
• Quality management of medical care, legal and administrative response for hospital operations
• Notification of new services and event information
• Notifications via SMS, email, LINE, WhatsApp, KakaoTalk, etc.
3. Disclosure to Third Parties
We do not share your personal information with third parties, except:
• When you have given prior explicit consent
• When required by law or by request from authorized governmental agencies
4. Processing Entrustment
We entrust the following service providers for operations:
• Vercel Inc. (hosting, USA)
• Supabase Inc. (database, USA)
• Slack Technologies (operational notifications, USA)
• LINE Yahoo (when entering via LINE, Japan / Korea)
Entrusted processing is limited to service operations and data is destroyed upon termination.
5. Retention and Use Period
Principle: Data is destroyed without delay after the purpose of collection and use is achieved.

• Consultation booking information: 5 years from submission date
• Cancelled submissions: 1 year from cancellation date
• Automatically collected information (access logs, etc.): 1 year
• Medical records (when actual treatment occurs): 10 years per Article 15 of the Enforcement Rule of the Medical Service Act
6. Destruction Procedure and Method
[Procedure] Information entered for booking application is destroyed without delay after the purpose is achieved.
